Having read, received and agreed to the Official KSU OPEN RULES ,completed the below entry form and paid the $100 per event, I/we agree to abide by all conditions stated therein. By signing this agreement, I/we hereby release the State of Ohio, FBFP 2012, KSU bass club, Rory Franks and any of it's officer’s, agents, employees and sponsors from any and all damages, claims, costs or expenses relating to injury or even death, of any persons, property, or character which we may sustain or cause by reason of our participation in or in connection with this tournament. I/we further authorize the use of any images or likeness of our person and names. I/ We have read and understand our agreements as outlined above.

Your participation supports the KSU collegiate Anglers on achieving their 2012 Goals:

-Introduce students to the sport of bass fishing, both recreationally and competitively

-Build friendships that will last a lifetime, especially with those who enjoy the sport of bass fishing

-Give students the opportunity to fish competitive tournaments, especially those who do not have access to a boat

-Find ways to educate the members and allow them to improve their                              knowledge and skills in bass fishing
